What to Include in a Referral

Patient Demographics

Full name, date of birth, phone number, and address.

Insurance Information

Insurance carrier, member ID, and group number.

Reason for Referral

Primary diagnosis, symptoms, and reason for psychiatric referral.

Clinical Notes

Relevant medical history, current medications, and prior treatments.

Records may be sent via fax at 737-221-5820. Do not email protected health information.

Provider Referral FAQ

Do patients need a referral to see a psychiatrist at KwikPsych?
No. Patients can contact KwikPsych directly to request an appointment without a referral. However, if a patient's insurance plan requires a referral or prior authorization, the referring provider should confirm this before sending the patient.
Who can refer a patient to KwikPsych?
We accept referrals from primary care physicians, psychiatrists, therapists, counselors, psychologists, licensed clinical social workers, hospitals, discharge planners, school counselors, EAPs, and case managers.
How do I submit a referral?
You can submit a referral by faxing patient information to 737-221-5820 or by calling 737-367-1230 to coordinate with our team. A secure online referral form is coming soon. For HIPAA compliance, please do not email patient health information.
What information should I include in a referral?
Please include the patient's full name, date of birth, contact information, insurance details, reason for referral, current medications, and any relevant clinical notes or prior evaluations.
How quickly will the patient be contacted?
We aim to contact referred patients within 1–2 business days of receiving the referral. For urgent or time-sensitive situations, please call 737-367-1230 so our team can prioritize scheduling.
Can I send records with the referral?
Yes. Sending relevant clinical notes, medication lists, and prior evaluations helps speed up intake and improves continuity of care. Please fax records to 737-221-5820.
Will I receive updates on my referred patient?
With proper patient consent and authorization, we are happy to coordinate care and share clinical updates with the referring provider. We can send progress notes by secure fax or through your organization's preferred secure channel.
